The request:

Montreal, February 14th, 2017

From: Lucie Aylwin, Director, Adélard-Desrosiers Elementary School
To: Fondation Coup de Coeur

Dear Madam Fatima Langlois and Mr. Jean Gagnon,

Following our telephone exchange, I became aware of the visual screening program that Fondation Coup de Cœur, under the direction of Dr Claude Chagnon Optometrist volunteer offers.
Considering this most relevant initiative, I would like to submit a screening request for my students.
Adélard-Desrosiers Elementary School, of the Pointe-de-l'Îe School Board, has 653 pupils from pre-kindergarten to grade 6. The school has a deprivation index of 10.
Thus, the family situation of the majority of our students involves several vulnerability factors, which increases the risk of academic difficulty and dropping out.
Many of our students do not receive an annual visual examination. In these circumstances, I would like to highlight the need for Adélard-Desrosiers to be among the schools that will have the opportunity to participate in this program.

Hoping for a positive response with great enthusiasm, I thank you for your interest in this request.
